Sequel Youth and Family Services (SYFS) strives to provide our employees with the most comprehensive benefits package possible at a cost that is affordable to our employee’s. As healthcare cost continues to increase, SYFS is committed to providing our employees and their families with resources to understand and effectively manage their benefits. We offer the following competitive options:
SYFS offers two medical plans through Blue Cross Blue Shield. The standard plan consist of a High Deductible Plan (HDHP) with a Health Savings Account (HSA) where Sequel makes annual contributions. The maximum plan consist of a low deductible with a $30 Physician’s office co-pay. Routine/wellness visits are covered 100% and prescription drug coverage is offered for both plans.
Similar to the medical plans, SYFS offers two dental plans through Cigna Dental. Preventive care for both plans are covered at 100% and the annual maximum deductible is $1000 or $1500 per member. Orthodontia is offered with a maximum of $2000 on the High Option plan for employees and their families (children under the age of 19).
VSP, one of the nation’s leading eyecare providers, is offered to our employees. With over 65,000 network providers, eye exams are covered every 12 months, frames covered up to $150 (every two years) and contact lenses covered up to $130 (every 12 months).
Employer Funded Insurances
SYFS offers a $25,000 Accidental Death & Dismemberment policy, Short-Term Disability (30% of weekly earnings up to $1000 max benefits) and Long-Term Disability (40% of monthly earnings up to $10,000 max). Employees have the option to purchase additional STD or LTD coverages.
Do you want to get paid on a daily basis? In 2018, SYFS partnered with DailyPay, a technology solution that allows our employees to control their gross wages when they get paid, allowing the opportunity of meeting financial goals and paying bills on time. For a minimal fee (same cost of going to an ATM machine), employees can select options on receiving their worked wages. A peace of mind, with funds available when they need it at the click of a button.
Seeing a doctor in the comfort of your home….Teladoc is a low-cost feature giving employees who enroll in one of SYFS medical plans access to a U.S. Board Certified network of doctors, psychiatrist and dermatologist at any time. Teladoc allows you to call or virtually connect with a doctor from home, work, or on the road allowing employees to avoid the hassle of sitting in a waiting room. Teladoc doctors diagnose non-emergency medical problems, recommend treatment, and can even call in a prescription to your pharmacy of choice.
